There are two versions of the notice:
- The Department of Managed Health Care (DMHC) has provided a Notice for members who cease coverage from a health maintenance organization (HMO) health plan.
- The California Department of Insurance (CDI) has provided a Notice for insureds who cease coverage from a preferred provider organization (PPO) insurance plan. Health Net will be sending the applicable notice to members who cease coverage. It will be included with their Prior Creditable Coverage letter.
